Acknowledgment of Risks
I, the Guardian, hereby acknowledge that I am voluntarily consenting to the Minor's participation in the bungee fitness class provided by Zing Bungee Fitness LLC. I understand that bungee fitness involves certain inherent risks, including but not limited to, falling, tripping, collision with objects or other participants, and potential injury, including serious bodily injury, disability, and death. I also understand that these risks may be caused by the Minor's own actions or inactions, the actions or inactions of other participants, the conditions of the premises, or the negligence of others, including Zing Bungee Fitness, its instructors, employees, agents, and representatives.
Assumption of Risk
In consideration of the Minor being allowed to participate in the bungee fitness class, I, the Guardian, on behalf of myself and the Minor, agree to assume all risks associated with this activity, including those risks caused by the negligence of others. I acknowledge that it is my responsibility to consult with a physician prior to the Minor participating in this class to ensure that the Minor is physically fit and able to engage in such activity.
I understand and agree that this Waiver and Release of Liability is binding upon myself, the Minor, and our respective heirs, executors, administrators, and assigns. I voluntarily waive any right to bring legal action against Zing Bungee Fitness for any injury or damages suffered by the Minor as a result of their participation in the bungee fitness class.
I, the Guardian, have read this Waiver of Liability, fully understand its terms, and voluntarily agree to its provisions. I confirm that I am the legal guardian of the Minor and have the authority to sign this Waiver on their behalf.
